In 1900 New York, the brilliant Dr. John Thackery takes over the head of the surgical department at Knickerbocker Hospital, known simply as "The Knick", after the sudden suicide of his mentor, J.M. Christiansen. Thackery, a renowned physician who works with innovative surgical techniques, is however plagued by cocaine addiction. The hospital, run by the daughter of the main financier, Cornelia Robertson, tries to remedy the debt by attracting wealthy patients, trying not to sacrifice the quality of care. Cornelia is responsible, despite Thackery's opposition, of the entry into the surgical team of Dr. Algernon Edwards, a black American surgeon trained at Harvard who trained in Paris who is far more qualified than any other candidate, must fight for the respect among all the staff of the "white" hospital, as well as in the racially motivated city. While struggling to keep the lights on, the hospital tries to attract a wealthy clientele, without sacrificing the quality of care.
